ABSTRACT

The retention of emergency patients is a common problem faced by hospitals worldwide. In addition to medical problems, social, family, economic and other problems will also cause patients to stay in the emergency department. It not only brings challenges to patients’ own disease treatment and hospital operation and management, but also brings ethical dilemmas. Taking the service cases of medical social workers who involved in stranded patients in Shanghai E Hospital as an example, this paper analyzed 20 service records and interviews with social workers receiving cases, and combed the ethical dilemmas faced by medical social workers in the process of involving in emergency stranded patients. It was found that medical social workers faced many ethical dilemmas in the process of involving in emergency stranded patients, such as interpersonal relations and relevant legal regulations, the right to life and health and the right to informed consent, the self-determination of case owners and the allocation of medical resources. In this regard, it is suggested to solve the problem of patient retention by advocating the improvement of relevant systems and policies, building cooperative teams to smooth referral channels, strengthening education, and promoting family shared decision-making.

ABSTRACT

To understand the current situation of occupational burnout, turnover intention, and professional self-care among social workers under the context of epidemic prevention and control, and the role of professional self-care in the relationship between occupational burnout and turnover intention, this study investigated 1 066 social workers in Fujian Province with Professional Self-Care Practice Scale, and the Burnout Scale. SPSS 26.0 and its PROCESS plug-in analysis were used, the results showed that the proportion of social workers with moderate and severe occupational burnout accounted for 44.22% and 29.99% respectively, the proportion with turnover intention in the past year accounted for 25.67%, and the average score of professional self-care was (22.57±5.98) points. There was a significant positive correlation between occupational burnout and turnover intention (r=0.29, P<0.01). Professional self-care of social workers was significant negatively correlated with occupational burnout (r=-0.46, P<0.01) and turnover intention (r=-0.16, P<0.01). Professional self-care played a moderating role in the relationship between occupational burnout and turnover intention (β=-0.01, P<0.05). This paper indicated that social workers had a high degree of occupational burnout and turnover intention, low level of professional self-care under the context of epidemic prevention and control, and the attention should be paid to the protective role of professional self-care.

ABSTRACT

Las intervenciones profesionales de los trabajadores sociales y los bibliotecarios durante la pandemia por COVID-19 convergieron en el empleo de las tecnologías de la información para satisfacer necesidades de información tanto de personal médico como de los pacientes y sus familias. La colaboración de estos dos profesionales reforzaría significativamente el equipo multidisciplinario de la salud a partir de la mejora de los canales de flujo de información que se llevan a cabo en el ámbito hospitalario, el fomento a la atención humanizada en salud y las acciones de inclusión social en las bibliotecas para brindar mayores y mejores oportunidades, así como recursos a población vulnerable.


The professional interventions of social workers and librarians during the COVID-19 pandemic converged in the use of information technologies to meet the information needs of both medical personnel and patients and their families. The collaboration of these two professionals would significantly strengthen the multidisciplinary health team by improving the information flow channels that are carried out in the hospital, promoting humanized health care and carrying out social inclusion actions in libraries to provide greater and better opportunities and resources for the vulnerable population.

ABSTRACT

Objective:To study the effect of medical social workers combined with ICU diary on the prevention and treatment of post-traumatic stress disorder in ICU traffic accident patients, and to explore the application of continuing nursing services and ICU diary to ICU traffic accident patients.Methods:A total of 47 patients who were admitted to the ICU of Suzhou Science and Technology City Hospital from January to December 2019 due to traffic accidents were selected by convenient sampling method. Twenty-three patients admitted to the hospital in the first 6 months were selected as the control group (before the introduction of medical social workers and ICU diaries) for routine ICU treatment and nursing mode. Twenty-four patients admitted to the hospital in the last 6 months were selected as the experimental group (after the introduction of medical social workers and ICU diaries), and the medical social workers combined with the ICU diary were intervened. The scores of the Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) were compared between the two groups of patients when they were admitted to the ICU, stayed in the ICU for a week, and transferred out of the ICU. The incidence of post-traumatic stress disorder was evaluated and compared by the scores of the Impact of Event Scale-Revised of the two groups of patients one month after leaving the ICU, three months after leaving the ICU, and six months after leaving the ICU.Results:The scores of SAS in the experimental group were 58.67 ± 7.45, 44.13 ± 5.47 and 39.08 ± 5.52 when they were admitted to the ICU, stayed in the ICU for a week, and transferred out of the ICU, and were 58.17 ± 6.75, 50.87 ± 6.35 and 42.74 ± 4.85, respectively in the control group. There was no statistical difference in the scores of the SAS between the two groups of patients when they were admitted to the ICU ( P>0.05), but there was a time difference between the two groups with time changes and different interventions ( Ftime = 110.98, P<0.01), there were also differences between groups ( Fbetween groups = 5.91, P<0.05). Compared with the control group, the SAS score of the experimental group decreased more significantly( Finteraction = 0.28, P<0.05). The scores of SDS in the experimental group were 57.75 ± 4.06, 45.29 ± 3.39 and 36.63 ± 3.49 when they were admitted to the ICU, stayed in the ICU for a week, and transferred out of the ICU, and were 57.48 ± 2.29, 49.04 ± 5.65 and 43.57 ± 4.07 respectively in the control group. There was no statistical difference in the scores of the SDS between the two groups of patients when they were admitted to the ICU ( P>0.05), but there was a time difference between the two groups of patients with time changes and different interventions ( Ftime = 248.24, P<0.01), there are also differences between groups ( Fbetween groups = 24.39, P<0.05). Compared with the control group, the SDS score of the experimental group decreased more significantly ( Finteraction=10.44, P<0.05). The incidences of post-traumatic stress disorder in the experimental group were 9, 8, 6 cases at one month, three months and six months after leaving the ICU, which were lower than 12, 11, 8 cases in the control group. The generalized estimation equation showed that the difference was statistically significant ( χ2=4.21, P<0.05). Conclusions:The intervention model of medical social workers combined with ICU diary is conducive to the use of ICU diary and the development of continuous nursing, which improves patients′negative emotions and reduces the incidence of post-traumatic stress disorder.

ABSTRACT

O objetivo da pesquisa é compreender a atuação do Assistente Social na alta hospitalar do Hospital Universitário Regional dos Campos Gerais (HURCG). O estudo é exploratório e de natureza qualitativa, sendo utilizados o formulário de dados e o grupo focal como procedimentos metodológicos de coleta de dados e, para interpretar os dados adquiridos no grupo focal, a análise de conteúdo temática. Constatou-se que a atuação do Assistente Social no HURCG perpassa os casos emergenciais e os complexos, sendo o vínculo de confiança e a resolutividade dos problemas os fatores determinantes na relação dos Assistentes Sociais com outros profissionais e com os pacientes na alta hospitalar no HURCG. O Assistente Social é considerado um dos profissionais essenciais no planejamento da alta hospitalar. Além disso, a articulação com a rede de atendimento e os princípios de humanização e integralidade no atendimento mostraram-se imprescindíveis nesse processo.


The research aims to understand the role of the Social Worker at the hospital discharge of the Hospital Universitário Regional dos Campos Gerais (HURCG). The study has an exploratory and qualitative nature. The data form and the focus group were used as methodological procedures for data collection. To interpret the data acquired in the focus group, thematic content analysis was used. It was verified that the role of the Social Worker in the HURCG pervades the emergency and complex cases and problem solving are the determining factors in the relationship between Social Workers and other professionals and patients on discharge from HURCG. It was noted that the Social Worker is considered as one of the essential professionals in planning hospital discharge. In addition, the articulation with the service network and the principles of humanization and integrality in care were essential in this process.


La investigación tiene como objetivo comprender la actuación del Asistente Social en el alta hospitalaria del Hospital Universitario Regional dos Campos Gerais (HURCG). El estudio posee un marco exploratorio y una naturaleza cualitativa. Como procedimientos metodológicos de recolección de datos se utilizaron el formulario de datos y el grupo focal. Para interpretar los datos adquiridos en el grupo focal se utilizó el análisis de contenido temático. Se constató que la actuación del Asistente Social en el HURCG atravesaba los casos de emergencia y los complejos, siendo el vínculo de confianza y la resolución de los problemas los factores determinantes en la relación de los asistentes sociales con otros profesionales y con los pacientes en el alta hospitalaria en el HURCG. Se notó que el Asistente Social es considerado como uno de los profesionales esenciales en la planificación del alta hospitalaria. Además, la articulación con la red de atención y los principios de humanización e integralidad en la atención se mostraron imprescindibles en ese proceso.

Resumen (analítico) Ante la escasez de evidencia, el objetivo general de este estudio fue describir y aplicar un instrumento propuesto para medir los derechos infantiles en Chile, desde el ejercicio cotidiano de operadores sociales que trabajan con infancia y adolescencia en centros educacionales, de salud, justicia o de desarrollo psicosocial. La muestra estuvo compuesta por 407 participantes (profesores, psicólogos, entre otros). El instrumento considera los cuatro principios fundamentales de la Convención de Derechos del Niño: no discriminación, interés superior, derecho a la vida, desarrollo, supervivencia y participación. Los resultados preliminares sugieren que existen diferencias estadísticamente significativas en el grado de cumplimiento de los derechos según el ámbito de desempeño. Se discute la consistencia in-terna del instrumento y su adaptación según ámbito, población atendida y aplicación en población infantil general y específica.


Abstract (analytical) Given the lack of evidence in this area, the general objective of this study was to describe and apply an instrument designed to measure children's rights in Chile based on the day to day practices of social operators who work with children and adolescents, in educational, health, justice and psychosocial development centers. The sample consisted of 407 participants (including teachers and psychologists). The instrument takes into account the four fundamental principles of the Convention on the Rights of the Child: non-discrimination; the best interest of the child; the right to life, development and survival; and the right to be heard. The preliminary results suggest that there are statistically significant differences in the degree of fulfillment of rights in relation to each fundamental principle. The internal consistency of the instrument, its adaptation to measure the fulfillment of each principle and the population that receive assistance are discussed, as well as application of the instrument with general and specific child populations.


Resumo (analítico) Dada a falta de evidências, o objetivo geral deste estudo foi descrever e aplicar um instrumento proposto para medir os direitos da criança no Chile, a partir da prática diária de operadores sociais que trabalham com crianças e adolescentes, em centros educacionais, de saúde, justiça ou desenvolvimento psicossocial. A amostra foi composta por 407 participantes (professores, psicólogos, entre outros). O instrumento considera os quatro princípios fundamentais da Convenção sobre os Direitos da Criança: Não discriminação; Interesse superior; Direito à vida, à sobrevivência e ao desenvolvimento; Participação. Os resultados preliminares sugerem que existem diferenças estatisticamente significativas no grau de cumprimento dos direitos de acordo com o âmbito de desempenho. Além disso, o trabalho discute a consistência interna do instrumento e sua adaptação segundo o campo, população atendida e aplicação em população infantil geral e específica.

Este artigo apresenta resultados contidos na dissertação de mestrado que buscou caracterizar o estado da arte acerca do trabalho do assistente social no Brasil recente. Este artigo relata a trajetória de construção da escuta-flânerie, um dispositivo de pesquisa-intervenção de caráter metodológico, cujas origens remontam à metodologia iniciada por Gurski (2008) em sua tese de doutorado, na qual foi construído o dispositivo do ensaio-flânerie. No encontro da Psicanálise com a socioeducação, tornou-se possível seguir os estudos que já vínhamos fazendo a partir da articulação da escuta psicanalítica com o tema da experiência e do flâneur em Benjamin e Baudelaire (Gurski, 2008, 2014; Gurski, & Strzykalski, 2018a, 2018c; Gurski, 2019). Escolher Benjamin e Baudelaire, em articulação com a escuta psicanalítica, a fim de traçar o caminho deste diálogo com o mal-estar na socioeducação implica compreender a contemplação do flâneur e da flânerie como um dispositivo que empresta uma posição possível ao pesquisador-psicanalista na Instituição. Ao nos colocarmos na posição de flâneur e oferecermos a possibilidade de uma fala livre e mais implicada com o tempo de cada um, dentro da instituição, evocamos o surgimento de uma narrativa mais próxima das questões do sujeito. Sustentar essa condição temporal comum ao psicanalista e ao flâneur tem se colocado como um desafio crescente na pesquisa. É desse modo que a flânerie, em associação à escuta, colabora com a teorização acerca da metodologia usada, enriquecendo nossos meios de falar da experiência e da construção desse espaço de intervenção e de pesquisa desde a Psicanálise na socioeducação.


This article reports on the construction of listening-flânerie, a research-intervention device of a methodological nature, whose origins go back to the methodology initiated by Gurski (2008) in his doctoral thesis, in which the device of the essay-flânerie was constructed. In the encounter between psychoanalysis and socio-education, it became possible to follow the studies that we had already done from the articulation of psychoanalytic listening with the theme of experience and flâneur in Benjamin and Baudelaire (Gurski, 2008, 2014; Gurski, & Strzykalski, 2018a, 2018c; Gurski, 2019). Choosing Benjamin and Baudelaire, in articulation with psychoanalytic listening, in order to trace the way of this dialogue with the malaise in the socioeducation implies understanding the contemplation of the flâneur and the flânerie as a device that lends a possible position to the researcher-psychoanalyst in the Institution. When we put ourselves in the position of flâneur and offer the possibility of a free speech and more implied with the time of each one, within the institution, we evoked the emergence of a narrative closer to the subject's questions. To sustain this temporal condition common to the psychoanalyst and the flâneur has been a growing challenge in research. It is in this way that the flânerie, in association with listening, collaborates with theorization about the methodology used, enriching our means of speaking of the experience and the construction of this space of intervention and research since Psychoanalysis in the socioeducation.


Cet article relate la construction de flânerie-écoute, un dispositif de recherche-intervention de nature méthodologique, dont les origines remontent à la méthodologie initiée par Gurski (2008) dans sa thèse de doctorat, dans laquelle le dispositif de l'essai-flânerie a été construit. Dans la rencontre entre psychanalyse et socio-éducation, il est devenu possible de suivre les études que nous avions déjà réalisées à partir de l'articulation de l'écoute psychanalytique avec le thème de l'expérience et du flâneur chez Benjamin et Baudelaire (Gurski, 2008, 2014; Gurski, & Strzykalski, 2018a, 2018c; Gurski, 2019). Choisir Benjamin et Baudelaire, en articulation avec l'écoute psychanalytique, afin de tracer la voie de ce dialogue avec le malaise dans la socio-éducation, implique de comprendre la contemplation du flâneur et de la flânerie en tant que dispositif permettant au chercheur-psychanalyste en institution de se positionner. Lorsque nous nous sommes mis en position de flâneur et avons offert la possibilité d'une parole libre et plus implicite au temps de chacun, au sein de l'institution, nous avons évoqué l'émergence d'un récit plus proche des questions du sujet. Soutenir cette condition temporelle commune au psychanalyste et au flâneur a constitué un défi croissant pour la recherche. C'est ainsi que la Flânerie, en association avec l'écoute, collabore à la théorisation de la méthodologie utilisée, enrichissant ainsi nos moyens de parler de l'expérience et de la construction de cet espace d'intervention et de recherche depuis la psychanalyse en socio-éducation.

ABSTRACT

O presente artigo pretendeu conhecer os níveis de stresse e engagement laboral que os assistentes sociais, em Portugal, estavam expostos. Para tal, aplicou-se um inquérito por questionário, composto pela Perceived Stress Scale e a Utrecht Work Engagement Scale, a 1369 profissionais. Os resultados identificaram um nível mediano de stresse e engagement, o que pode ser indicativo de níveis de bem-estar laboral satisfatórios. Observou-se, ainda, que à medida que o stresse percecionado aumentava, diminuía o engagement destes profissionais. Foram, também, encontradas conexões significativas entre estas variáveis e o vencimento auferido.


This article intended to know stress and engagement work levels that social workers in Portugal were exposed to. A survey was answered by 1369 professionals. Perceived Stress Scale and Utrecht Work Engagement Scale were used in the construction of the questionnaire. The results point to a median level of work stress and engagement, which may be a sign of satisfactory levels of well-being. We also found that as perceived stress increases the engagement of these professionals decrease. Significant connections between these variables and the earned salary emerged.


El presente artículo pretendió conocer los niveles de estrés y engagement que los trabajadores sociales em Portugal fueran expuestos. Para ello, se aplicó una encuesta por cuestionario, compuesto por la Perceived Stress Scale y la Utrecht Work Engagement Scale, a 1369 profesionales. Los resultados identificaron un nivel mediano de estrés y engagement, lo que puede ser indicativo de niveles de bienestar laboral satisfactorios. Se observo, además, que a medida que el estrés percibido aumentaba, disminuía el engagement de estos profesionales. También se encontraron conexiones significativas entre estas variables y el vencimiento obtenido.

ABSTRACT

Diabetes is a chronic disease that requires patient self-management. Successful self-care by diabetics includes accepting their diabetes status and learning proper diabetes management, as well as creating and maintaining a lifestyle that enables diabetes management.Recently, in order to improve diabetics’ self-care capability, health education has been shared by trained specialists (doctors, nurses, nutritionists, and medical social workers, etc.) through team access. Under these circumstances, the medical social worker acts as a member of the treatment team and plays a role in helping patients’ stabilize treatment and return to society smoothly by training them in psychological, economic, and social issues that make living with diabetes treatment difficult.The purpose of this study was to examine the role of medical social workers engaged in the treatment of diabetics based on their clinical work.

RESUMEN Objetivo: describir el nivel del soporte social percibido por los usuarios de los servicios de salud de segundo nivel. Método: estudio descriptivo, con una muestra de 287 usuarios de los servicios de salud del estado de Durango, México, aplicando la Escala de Soporte Social de Salud, utilizando en programa SPSS para el análisis de resultados. Resultados: se encontró que el usuario de la consulta externa del hospital, percibe mayor soporte social por parte del personal de enfermería en comparación con el médico y trabajo social, presentando una media de (M=10.70, 10.19, 9.58) respectivamente; con una diferencia significativa de p<0.05. Conclusión: es el personal de enfermería el que brinda mayor soporte social a los usuarios de los servicios de salud, resolviendo cada una de las dudas que se les presenten y permitiéndoles expresar sus sentimientos.


RESUMO Objetivo: Descrever o nível de suporte social percebido por usuários de serviços de atenção secundária. Método: Estudo descritivo com uma amostra de 287 usuários de serviços de saúde do estado de Durango, México, aplicando a Escala de Suporte Social de Saúde, e utilizando o programa SPSS para a análise de resultados. Resultados: Os usuários ambulatoriais do hospital perceberam maior suporte por parte da equipe de enfermagem quando comparado aos médicos e assistentes sociais, com uma média de M=10,70, 10,19, e 9,58, respectivamente; e com uma diferença significativa de p<0,05. Conclusão: A equipe de enfermagem fornece mais suporte social aos usuários dos serviços de saúde, respondendo às perguntas que são feitas e permitindo aos pacientes que expressem seus sentimentos.


ABSTRACT Objective: To describe the level of social support perceived by users of a second-level health service. Method: This was a descriptive study with 287 users of health services in the state of Durango, Mexico, using the Social Support Scale in Health to collect data and SPSS software to analyze the results. Results: The outpatients perceived more social support from nursing staff than from physicians and social workers, with a mean of 10.70, 10.19, 9.58, respectively; with a p<0.05 significant difference. Conclusion: Nursing professionals provide health service users with the greatest social support, answering their questions and allowing them to express their feelings.

ABSTRACT

En México como en el resto de Latinoamérica nuestros hábitos alimenticios nutricionales, culturales y educación favorecen a los factores de riesgo para desarrollar padecer diversas enfermedades crónicas degenerativas como la diabetes, hipertensión arterial, obesidad o trastornos como el aumento de colesterol y triglicérido. Cuando tres o más de los anteriores factores se presentan en conjunto proporcionan lo que conocemos como síndrome metabólico. El objetivo de este estudio fue determinar la prevalencia del síndrome metabólico de estudiantes con factores de riesgo para desarrollarlo en estudiantes universitarios. El estudio se realizó a estudiantes de primer año de la Universidad Veracruzana Campus Minatitlán, tomando en cuenta los resultados del Examen de Salud Integral que se les aplico al ingresar a la universidad, además se les realizó una prueba rápida de glucosa, somatometría y un cuestionario sociodemografico. Se estudió una población de 91 estudiantes de las facultades de Medicina, Odontología y Trabajo social de los cuales 73,6 por ciento eran mujeres y 26,4 por ciento hombres, en una edad promedio de 19.2 ± 1.1 años, encontrando una prevalencia de 6,6 por ciento de síndrome metabólico. Además se observó que 37,4 por ciento presentaron un factor de riesgo, 12 por ciento dos factores de riesgo y 44 por ciento no mostraron ningún factor de riesgo. Siendo el principal factor de riesgo la obesidad presente en 44 por ciento de los estudiantes, seguido de las hipertrigliceridemias. La prevalencia de síndrome metabólico en los estudiantes de nuevo ingreso de la facultad de medicina del campus Minatitlán es alta con respecto a los reportes nacionales e internaciones, en donde más del 50 por ciento presenta algún factor de riesgo, siendo la obesidad el principal con un mayor predominio en las mujeres(AU)


In Mexico as the rest of Latin America, our nutritional habits, cultural and educational favor to risk factors for developing chronic degenerative diseases such as diabetes, high blood pressure, obesity or disorders such as cholesterolemia, increased cholesterol and triglyceride. When three or more of the above factors are presented together provide which we know as metabolic syndrome. The objective of this study was to determine the prevalence of metabolic syndrome and risk factors for developing in university students. The study was conducted to first year students of the Health Sciences and Social Work at the Universidad Veracruzana campus Minatitlán, taking into account the results of the Integral Health Examination that were applied to them when entering the university, also was performed as a rapid glucose test, somatometry, and a socio-demographic questionnaire. A population of 91 students from the Faculty of Medicine, Dentistry and Social Work were studied, of which 73,6 percent were women and 26,4 percent were men, with a mean age of 19,2 ± 1,1 years, where 6,6 percent were found to have metabolic syndrome. We also observed that 37.4 percent had a risk factor, 12 percent had two risk factors and 44 percent did not show any risk factors. Being the main risk factor the obesity present in 44 percent of the students, followed by hypertriglyceridemia. The prevalence of metabolic syndrome in the newly enrolled students of the Health Sciences Area and Social Work of the Minatitlán campus is high with respect to national and international reports, where more than 50 percent presents some risk factor, being obesity the main one in a greater predominance in women(AU)

ABSTRACT

In diabetes camps, medical social workers interact with individuals and groups of participants to help them learn to adapt psychologically and emotionally to the stress from diabetes and to improve their ability to cope with interpersonal relationships. The change stage model (transtheoretical model) is applied to change the behavior of diabetics. Social worker intervention programs in diabetes camps provide opportunities for patients to manage their diabetes healthily by increasing self-esteem, participating in motivational counseling, and developing stress management, fear and conflict resolution. In particular, in the 2030 diabetes camp program, young diabetics can learn the personal importance of medical social workers through the ‘post-camp’ talk.

Este artigo, ao dar contorno a alguns dos achados de estudos e pesquisas que caminham em regime de coprodução em diferentes atividades de formação junto a profissionais, grupos e integrantes de movimentos sociais, reafirma o lugar tensionado que assumem os denominados trabalhadores sociais em nosso presente. Tomando como campo de nossas análises os processos de judicialização da vida colocados em andamento no cenário contemporâneo, refletimos sobre como determinadas demandas e políticas promotoras dos direitos humanos operam. Imersas nas lutas por igualdade e justiça, nos move a crença de que a questão humana deve prescindir de fórmulas simplificadoras. Entendemos que o exercício do direito ganha outras tensões, já não se tratando apenas de reivindicação endereçada ao Estado, mas de todo um campo de disputas, de litígios que se instala nas relações de vizinhança - entre grupos, indivíduos, instituições, entre movimentos sociais


This article, by giving shape to some of the study findings and researches that go together with a co-production regime in different training activities along with professionals, groups, and members of social movements, reaffirms the tense place assumed by the designated social workers in our present. Taking the processes of life judicialization in the contemporary scenario as the basis for our analysis, we reflect on how certain demands and policies promoting human rights operate. Immersed in the struggles for equality and justice, we are moved by the belief that the human question should dismiss simplifying formulas. We understand that the exercise of law generates other tensions since it is no longer a claim addressed only to the State, but rather a field of disputes and litigations, which establishes itself in neighborly relations - between groups, individuals, institutions, among social movements


Este artículo, al dar contorno a algunos de los hallazgos de estudios e investigaciones que caminan en régimen de coproducción en diferentes actividades de formación junto a profesionales, grupos e integrantes de movimientos sociales, reafirma el lugar tensado que asumen los denominados trabajadores sociales en nuestro presente. Tomando como campo de nuestros análisis los procesos de judicialización de la vida puestos en marcha en el escenario contemporáneo, reflexionamos sobre cómo ciertas demandas y políticas promotoras de los derechos humanos operan. Inmersiones en las luchas por igualdad y justicia, nos mueve la creencia de que la cuestión humana debe prescindir de fórmulas simplificadoras. Entendemos que el ejercicio del derecho gana otras tensiones, ya no se trata sólo de reivindicación dirigida al Estado, sino de todo un campo de disputas, de litigios que se instala en las relaciones de vecindad - entre grupos, individuos, instituciones, entre movimientos sociales

Resumo Este artigo problematiza a singularidade da desigualdade social brasileira, produtora de uma 'ralé' estrutural e legitimada por uma hierarquia valorativa que separa pessoas em gente e subgente, cidadãos e subcidadãos; pessoas que devem ser vistas e respeitadas, de um lado, e pessoas desdenhadas, desvalorizadas, de outro. Posteriormente, abordamos como acontece a reprodução do esquema da desigualdade social na saúde pública, considerada de péssima qualidade e que contradiz, na prática, os preceitos constitucionais pelos quais o Sistema Único de Saúde foi criado, não beneficiando a proposta idealizada em seu cerne de garantir cidadania inclusiva e igualitária. Finalmente, propomos reflexões em torno da atuação dos assistentes sociais na saúde pública a partir do atendimento direto aos usuários, principalmente por meio das ações educativas, considerando-as como uma oportunidade para trabalhar, com a 'ralé', aspectos que mantêm tal classe exatamente numa situação de subcidadania e, por isso, receptora de atendimento desumano nos serviços públicos de saúde.


Abstract This article discusses the uniqueness of Brazilian social inequality, which produces a structural 'rabble' legitimized by a value hierarchy that segregates people into people and sub-people, citizens and sub-citizens; people who must be seen and respected, on the one hand, and people to be disdained, devalued on the other. Subsequently, we discuss how the social inequality scheme is reproduced in public health, which is considered of poor quality and, in practice, contradicts the constitutional precepts pursuant to which the Unified Health System was created, not benefiting the idealized proposal at its core to ensure inclusive, egalitarian citizenship. Finally, we propose reflections about the role social workers play in public health through the provision of direct service to users, especially through educational actions, considering them as an opportunity to work with the 'rabble,' aspects that maintain such class exactly in a situation of sub-citizenship and, therefore, receiving inhumane care from public health services.


Resumen Este artículo problematiza la singularidad de la desigualdad social brasileña, productora de una 'plebe' estructural y legitimada por una jerarquía valorativa que separa personas en gente y subgente, ciudadanos y subciudadanos; personas que deben ser vistas y respetadas, por un lado, y personas despreciadas, desvalorizadas, por otro. Posteriormente, abordamos cómo tiene lugar la reproducción del esquema de la desigualdad social en la salud pública, considerada de pésima calidad y que contradice, en la práctica, las premisas constitucionales sobre las cuales se creó el Sistema Único de Salud, no beneficiando la propuesta idealizada en su esencia de garantizar ciudadanía inclusiva e igualitaria. Finalmente, proponemos reflexiones en torno a la actuación de los asistentes sociales en la salud pública a partir de la atención directa de los usuarios, principalmente por medio de acciones educativas, considerándolas como una oportunidad para trabajar, junto a la 'plebe', aspectos que mantienen tal clase exactamente en una situación de subciudadanía y, por eso, receptora de atención deshumana en los servicios públicos de salud.

ABSTRACT

The paper introduced the present full-time practice of medical social workers in China, with in-depth analysis of the roadblocks encountered.The problems include but are not limited to multiple stakeholders without a healthy resources relationship network, lack of powerful government support in policy and resource orientation, absence of drivers for hospitals to deliver social service, immature social organizations, and poor capacity to link resources.Based on such, the authors explored the establishment of a development model for full-time medical social workers on a multiple cooperation platform, in such perspectives as the government, social worker organizations, hospitals, and third-party performance appraisal.

The family doctor service is an important measure to transform primary health care mode,makes the community health service institutions to take on the gatekeeper role of residents' health and health care costs,and promotes the achievements of hierarchical diagnosis and treatment.At present,there are some difficulties in the process of family doctor service.For example,the construction of the general practitioner team is lagging behind and the residents don't understand the specific contents of the family doctor service well.Medical social workers have both the social work professional skills and medical literacy,which makes them become the effective strength of community health service.Introducing medical social workers by the purchase of the governments,volunteer training,and job settings mode can meet the needs of the development of family doctor service to some extent,and bring in new ideas to the implementation of the family doctor service.

Resumo: As práticas de assistentes sociais e psicólogos no campo sociojurídico capixaba contam histórias que desvelam tanto os modos de funcionamento instituídos naquele espaço laboral, quanto provocam a desestabilização do saber/poder daquelas categorias profissionais, descortinando as formas de resistência que têm sido forjadas cotidianamente em favor de uma postura ético-política de defesa da vida, na contramão de sua recorrente judicialização, como o investimento no trabalho em rede.


Abstract: The social workers and psychologists´ practice in the social-judiciary field in Espírito Santo is related to stories that both show how work is done in that space, and lead to the destabilization of the knowledge-consent of those professional categories, thus uncovering ways of resistance that have been invented in favor of an ethical-political attitude defending life, on the opposite way of its repeating judging. Networking is an example of that.

El artículo despliega diversas consideraciones conceptuales sobre el Trabajo Social y formula distintas propuestas para ser tenidas en cuenta en la construcción de una suerte de posible agenda para la profesión en América Latina. Aborda aspectos referidos a la formación profesional de los trabajadores sociales, como así también lo inherente al ejercicio profesional en las instituciones y a la organización de la categoría profesional.
